CentraCare is a program designed to keep the frail elderly out of a nursing home with the belief that most participants would prefer staying in their own homes as long as possible. If the time comes when you need nursing home care, CentraCare will arrange for care with a nursing home that is within CentraCare's provider network.  CentraCare will continue to manage your care.
