Enrollment
If you or somebody you know may benefit from becoming a CentraCare participant, we invite you to review the information below to determine eligibility for the program and payment information and then visit the Enrollment Form to request more information. Participant Eligibility
Potential participants in the CentraCare program must meet the following criteria:

• Be 55 years of age or older
• Live within Calhoun County
• Meet nursing home level of care as determined by State of Michigan guidelines 
• Determined to be able to live safely in the community

Payment for Services
CentraCare receives monthly payments from Medicare and Medicaid to cover services for eligible participants. Most participants pay nothing.

PACE Lock-in Feature
As a CentraCare participant, all health care services are provided and arranged by your personal care team. PACE participants may be fully liable for the costs of medical services from an out-of-network provider or without prior authorization with the exception of emergency services.
