Keeping seniors at home
When asked, most older adults likely would say that they want to live in their own communities as long as they can.
For some, with good health, a solid support system and adequate resources, this dream is a reality.
For others, with chronic health issues, few family or community ties or limited funds, staying in the community is more of a challenge.
Often the solution is going to a nursing facility. This solution addresses the person's health needs, but not the sense of security she or he gains from staying at home among familiar surroundings with a community support network.
That's why CentraCare has been established in Calhoun County - to meet the social and health care needs of frail older adults. Through its Day Health Center, which may be visited one to several days a week, supported by any necessary outside services, CentraCare helps keep its participants in their homes.
With an individualized plan of care, specific needs are met for each participant. He or she has peace of mind and the community is stronger for having its seniors at home.
CentraCare is part of the National Program of All Inclusive Care for the Elderly (PACE ) Association. It is a medical/social-based model of care that provides comprehensive integrated managed care to older adults who are at risk for nursing home placement. Through interdisciplinary team planning and coordination, frail elderly adult participants receive seamless care as they age in place at home. All services and products covered through Medicare and Medicaid are provided by the PACE organization.
